Client intake paperwork

There are three confidential forms to complete before your first session, so Jenny can review your history and support you well from the start. Most people finish them in about half an hour. If you are joining one of Jenny's groups, there is one more.

Complete it online

The first three are required for everyone. The fourth is only for group members. Everything submitted here syncs directly to Jenny's HIPAA-compliant clinical drive.

Prefer paper? Everything is here. Print them, then scan or photograph the completed pages and email them to [email protected] before your first session.

The first two are questionnaires to fill in. The disclosure statement is for you to read and keep, and only its last page needs signing and returning. The group agreement is only for group members, and is signed at the end.
